If found susceptible to rubella, the Practitioner must be immunized; (Titled: MMR Acceptance Declination Form)Immunity to rubeola. If found susceptible to rubeola, the Practitioner must be immunized; (Titled: MMR Acceptance Declination Form)Immunity from varicella (chickenpox). If found susceptible to varicella, the Practitioner must be immunized; (Titled: Acknowledgement of Varicella)Immunity to mumps. If found susceptible to mumps, the Practitioner must be immunized; (Titled: MMR Acceptance Declination Form)Immunity to pertussis. If found susceptible to pertussis, Contractor must provide documentation that the immunization process is in progress or completed, or a copy of the Practitioner's signed declination; (Titled: T-Dap-Tetanus/Dipth/Pertussis Immunization)Immunity to hepatitis B.
