Provide hands-on care: assist clients with personal hygiene, mobility, light exercise, meals, housekeeping, errands, and medication reminders. Ensure quality & safety: complete home-safety assessments, conduct routine quality assurance visits, and document plan-of-care updates for the Care Manager. Train & onboard caregivers: deliver one-on-one coaching, skills refreshers, and ongoing competency checks. Serve on the on-call rotation: triage caregiver call-outs, fill urgent shifts, and resolve client concerns after hours. Problem-solve in real time: address changes in client condition, elevate issues, and keep families informed. What You’ll Bring 5+ years of professional caregiving, HHA, CNA, or home-health experience (lead/supervisory duties a plus) Proven ability to coach peers and communicate effectively with clients, families, and clinical staff Reliable transportation, valid driver’s license, and ability to lift/transfer clients as needed Tech-savvy enough to document care notes in a mobile app or EMR Compassionate, ethical, and calm under pressure - professional in every scenario Ready to Lead with Heart?
