Clinical Pharmacist

Description Northwest Kidney Centers seeks a Clinical Pharmacist to join our organization. The position will be based in our Burien location in a Part-Time .8 FTE, Monday-Friday 8am-4:30pm The Northwest Kidney Center Pharmacy is an outpatient retail-clinic pharmacy that provides pharmacy services and pharmaceutical care to chronic kidney disease patients, kidney dialysis patients, and kidney transplant patients. Founded in 1962 as the world's first outpatient dialysis provider , Northwest Kidney Centers operates 19 dialysis clinics across the Puget Sound region in addition to hospital-based services and a large home-based dialysis program. We are a not-for-profit, community-based organization and consistently rank among the top dialysis providers in the United States in quality of care, patient outcomes and transplant rates. Responsibilities The Clinical Pharmacist will fill and dispense prescriptions using sound professional judgment and clinical expertise. In addition, the clinical pharmacist will provide medication counseling, medication reconciliation, Medication Therapy Management, and perform drug therapy monitoring to resolve medication-related problems and support optimal health outcomes. The pharmacist will also provide drug information to dialysis clinic staff and providers. In addition, the clinical pharmacist will assist in making business decisions and in the day-to-day operation of the pharmacy.
