PRN Occupational Therapist (OT)
Aventura at Humility House | Austintown, OH
Aventura at Humility House is seeking a compassionate and dedicated PRN Occupational Therapist (OT) to join our in‑house rehabilitation team. This flexible opportunity is ideal for a clinician who enjoys working in a Skilled Nursing Facility/Long‑Term Care (SNF/LTC) environment and is committed to helping residents achieve their highest level of independence and function.
Responsibilities:

Evaluate and treat residents in accordance with physician orders and established care plans.

Develop individualized treatment programs focused on maximizing independence and quality of life.
-
Complete timely and accurate documentation utilizing Net Health and PointClickCare (PCC) .
-
Collaborate with therapy, nursing, and interdisciplinary team members to ensure exceptional patient outcomes.
-
Maintain compliance with all facility, state, and federal regulations.
Qualifications:
-
Current Ohio Occupational Therapist license in good standing.
-
SNF/LTC experience preferred, but not required.
-
Experience with Net Health and PointClickCare (PCC) preferred.

Strong communication and organizational skills.
-
Commitment to providing high‑quality, resident‑centered care.
