Role Description The MDS Coordinator is a full-time, on-site role based in Putnam, CT, responsible for managing the Resident Assessment Instrument (RAI) process and ensuring accurate and timely completion of Minimum Data Set (MDS) assessments. Day-to-day tasks include coordinating interdisciplinary input for assessments, reviewing clinical documentation, and entering data to support appropriate care planning and reimbursement. The role involves monitoring regulatory changes related to MDS, ensuring compliance with state and federal requirements, and collaborating with nursing, therapy, social services, and administrative staff to align assessments with residents’ clinical needs. The MDS Coordinator also participates in care plan meetings, provides education to staff on documentation and assessment standards, and assists with quality improvement initiatives and audits. Qualifications Strong clinical assessment and care planning skills, with experience reviewing nursing and therapy documentation for accuracy and completeness. Knowledge of MDS, RAI, and related regulatory requirements, including Medicare and Medicaid reimbursement guidelines in skilled nursing settings. Proficiency with electronic health records (EHR) and data entry, with attention to detail and ability to analyze resident data for trends and quality outcomes. Effective communication and collaboration skills to work with interdisciplinary teams, residents, and families in a respectful, person-centered manner. Organizational and time-management skills to handle multiple assessments, deadlines, audits, and regulatory updates. Current Registered Nurse (RN) or Licensed Practical Nurse (LPN/LVN) license in good standing in the state of Connecticut. Prior experience as an MDS Coordinator or in long-term care, skilled nursing, or rehabilitation settings strongly preferred. Familiarity with CMS regulations, quality measures, and care transitions; experience supporting quality improvement projects is beneficial.
