Schedule: Full-Time | MondayFriday, 8:00 AM 5:00 PM | 1 Weekend Per Month | 1 On-Call Shift Per Week

Compensation: $85,000 $130,000 annually plus $6,000 in first-year bonuses

What You'll Do Manage a home health caseload throughout your assigned territory. Complete OASIS assessments and individualized care plans. Deliver skilled nursing care and patient education. Collaborate with physicians and interdisciplinary team members. Help patients remain safe, healthy, and independent at home. Maintain timely and accurate clinical documentation.

What We're Looking For Active Minnesota Registered Nurse (RN) license. 3+ years of RN experience (home health experience preferred). Strong assessment, communication, and organizational skills. Ability to work independently while managing a field-based schedule. Reliable transportation.
