Requirements

• Iowa Mandatory Reporter Certificates
• State License Details: Must Be Currently Active
• Proof of physical within 1 year
• Nursing Certifications (TNCC)
• 10 Panel Drug Screen
• Resume
• All copies of required certifications: BLS, ACLS, PALS, TNCC
• Two recent employment references
• OIG checks
• Criminal Background check
• Immunizations: MMR, Varicella, Polio, Hepatitis B, TDaP, Flu vaccination
• Negative 2-step Mantoux or QuantiFERON Gold
• General Certifications (ACLS, BLS/BCLS, PALS)
• Completion of CareLearning education modules
• Respiratory Fit Test
• National and/or State Sex Offender checks
• Online verification of licensure or certification
