Seeking a compassionate Hospice RN Case Manager to provide comprehensive end-of-life nursing care, develop individualized care plans, and coordinate services for hospice patients and their families.
Job description The Hospice RN Case Manager plays a vital role within a multidisciplinary team by providing expert nursing care and support to hospice patients and their families. This position focuses on comprehensive patient assessments, care planning, and coordination to ensure compassionate end-of-life care, symptom management, and adherence to compliance standards. Responsibilities- Conduct thorough patient assessments to determine care needs
- Develop and implement individualized care plans
- Manage medications ensuring proper administration and monitoring
- Monitor patient symptoms and adjust care as necessary
- Provide counseling and emotional support to patients
- Support family members through education and guidance
- Coordinate care among healthcare providers and services
- Maintain accurate and comprehensive documentation
- Collaborate with interdisciplinary team members for holistic care
- Ensure compliance with hospice care regulations and standards
- Advocate for patients to uphold their rights and preferences
- Associate Degree in Nursing (ADN)
- 1+ years of experience in hospice nursing
