Community Health Guide: Field Outreach & Care Navigation
Reema Health, Inc.
Reema Health, Inc. seeks a Community Health Navigator to engage with members, enroll them in our care model, and coordinate care planning in a field-based, startup environment in the Tacoma area. You will travel 25–50% within a 30 mile area, build relationships, document care plans in Reema's platform, and work with a multidisciplinary team to address social determinants of health.
