Licensed Practical Nurse (LPN)

A 13-week licensed practical nurse (LPN) role in long-term care, based in or near Hendricks, Minnesota. The LPN delivers evening nursing care to residents, monitors vital signs, and collaborates with the interdisciplinary team to implement individualized care plans. This position supports the organization's mission to provide high-quality, compassionate care to long-term residents.

Key Responsibilities:

Provide high-quality nursing care to LTC residents during evening shifts
Assess patient conditions, monitor vital signs, and document findings accurately in electronic health records
Collaborate with the interdisciplinary team to develop and implement individualized care plans
Administer medications and treatments as prescribed, adhering to safety protocols and regulatory standards
Educate residents and their families regarding health management and post-discharge instructions

Qualifications:

Valid LPN license or equivalent certification recognized in Minnesota
CPR certification
First Aid certification
At least 1 year of recent LTC LPN experience
Ability to work evenings
Excellent communication skills and adaptability to various clinical settings
