Pharmacy Medication Reconciliation Technician

Schedule: Varied Shift, 8 hours, 40 per week

Assignment Details: Guaranteed Hours: 40 Contract Length: 13 weeks

Roles & Responsibilities:

• Obtain medication histories from patients.
• Assist with the reconciliation of home medications with those prescribed during hospitalization.
• At discharge, identify and communicate medication discrepancies to prescribers for reconciliation.
• Perform standard and structured tasks related to medication management.
