Job Overview
We are seeking a compassionate and dedicated Direct Support Professional to join our team. In this role, you will provide essential support to individuals with disabilities or special needs, helping them achieve greater independence and improve their quality of life. Your focus will be on delivering personalized care and assistance in various settings, including home care, nursing homes, and community environments.

Responsibilities

• Assist clients with daily living activities, including personal hygiene, meal preparation, and medication administration.
• Monitor and document patient progress and changes in behavior or health status.
• Implement care plans tailored to each individual's needs, ensuring that all goals are met effectively.
• Foster a supportive and encouraging environment by promoting social interaction and community engagement.
• Provide patient observation to ensure safety and well-being at all times.
• Collaborate with healthcare professionals, family members, and social workers to coordinate care efforts.
• Maintain accurate records of services provided and report any concerns to appropriate personnel.

Skills

• Strong patient observation skills to identify changes in behavior or health conditions.
• Experience in home care or nursing home settings is preferred.
• Knowledge of patient monitoring techniques and medication administration protocols.
• Familiarity with long-term care practices and social work principles is advantageous.
• Ability to develop and implement effective care plans based on individual needs.
• Excellent communication skills to interact with clients, families, and team members effectively.
