Community Health Navigator
Community Care of West Virginia
Community Care of West Virginia Inc is seeking a Referral Coordinator to manage the intake and processing of behavioral health referrals. You will verify patient demographics, insurance eligibility, and registration information, and enter referrals into the EHR while ensuring completeness and timely progression through each stage. The role requires coordinating with referral specialists, assessment clinicians, therapists, school personnel, and external agencies to place patients, maintain #J-18808-Ljbffr
