Care Team Manager – Reema Health Reema Health is a social care and behavioral health provider dedicated to a personalized, community‑based approach to care. The Care Team Manager plays a pivotal role in leading a hybrid care team that partners with clinical psychotherapists and prescribers to deliver holistic support for members living with serious mental illness and substance use disorder. Job Overview Work Schedule: Monday–Friday with occasional evenings and/or weekends. Vehicle Required: Yes. Travel: Occasional domestic travel in Indiana and to Minneapolis, MN. Reports To: Assistant Director of Care Team Operations. Positions Supervised: Behavioral Health Community Guides. Responsibilities Lead the Care Team Program with 90‑95% focus on leadership responsibilities such as setting metrics, managing performance, and driving change management in a fast‑paced startup environment. Supervise 5‑10% of direct service activities with members, including cold outreach, crisis support, and referral coordination through texting, phone, home visits, and face‑to‑face communication. Partner with clinicians to align services, manage multidisciplinary workflows, and collaborate on ensuring high quality service delivery. Conduct weekly check‑ins, staff and metrics updates, and monthly Care Team goal setting with the Director of Clinical Care Team Operations. Maintain confidentiality of PHI, HIPAA compliance, and software security policies. Support hiring, onboarding, ongoing training, and development of Care Team staff. Establish and sustain relationships with local community partners across the mental health continuum of care. Education, Experience, and Skills Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or related field (or equivalent experience). Three years of people management/supervision experience in healthcare, mental health, or community‑based settings. Five years of direct experience with the local population served and knowledge of the community. Proficiency in critical thinking, strategic thinking, time management, and prioritization. Knowledge of crisis planning, case management, and behavioral health. Experience working with individuals from diverse ethnic backgrounds, serious and persistent mental illness, chemical health needs, and the belief that recovery is possible. Experience working on a multidisciplinary team alongside clinical and non‑clinical staff. Experience working with adolescents and their families, and with difficult to reach populations, including homeless individuals. Proven ability to navigate ambiguity, creatively problem‑solve, build relationships, and execute outreach plans in fast‑paced environments. Working knowledge of Microsoft Word, Outlook, Excel, and Video Conferencing; G‑Suite preferred. Qualifications & Position Requirements 18 years of age or older. Valid Driver’s License, vehicle insurance, and satisfactory driving record. Pre‑employment drug screening and background check required. Compliance with federal, state, and local regulations regarding patient confidentiality and HIPAA. Daily use of technology‑based tools and systems. Lift and carry up to 25 pounds occasionally.
