Transitions Case Manager (MSW) | Patient & Family Care Advocate
Boston Medical Center
Boston Medical Center is seeking a dedicated Social Worker to assess patients' appropriateness for program admission and coordinate care plans that meet patient needs. This role includes conducting in-home assessments and ensuring proper documentation of client interactions. Required qualifications include a Master's Degree in Social Work and two to four years of relevant experience, preferably with hospice populations. A passion for supporting clients and strong communication skills are essential.
