Registered Nurse (RN) – Home Health Location: Fort Lee, Virginia (and Colonial Heights, VA) Employment Type: Staff – Part‑time or Full‑time, Ongoing. Job Overview Provide home health nursing services to clients in senior living communities. Conduct home visits, administer medications, perform wound care, and coordinate care with clinical and client services staff. Responsibilities Follow designated care plan in accordance with patient’s needs. Make home visits to clients within assigned geographic territories. Perform assigned duties including medication administration, wound care, treatments, and procedures. Monitor client conditions and report changes to Clinical or Client Services Manager. Follow up, execute, and document doctors’ orders accurately. Perform client assessments as needed. Case management and coordination of care. Accurately document observations, interventions, and evaluations via the state‑of‑the‑art tablet. Qualifications Current Registered Nurse license in Virginia. Minimum one year of recent, verifiable clinical experience (medical/surgical). Home‑care experience preferred but not required. Graduation from an accredited nursing program (transcript or diploma). Ability to work independently and manage time effectively. Strong interpersonal communication skills. Solid computer proficiency; prior EMR experience preferred.
