On Call Visiting Nurse

As an On Call Visiting Nurse at Hartford Healthcare, you will provide after-hours and urgent in-home nursing care to diverse patients across our network. Responsibilities include triage, comprehensive assessments, medication administration, wound care, care coordination, and timely documentation in the EMR. You will collaborate closely with interdisciplinary teams to ensure safe discharges, prevent avoidable hospitalizations, and support patients and families during complex or acute episodes.
