Job Summary

We are seeking an experienced and motivated Home Care Marketer / Community Liaison with strong experience growing both Medicaid and private-pay home care census. The ideal candidate will have established relationships within the healthcare and community network and a proven track record of generating referrals from hospitals, rehab centers, assisted living facilities, physicians, social workers, case managers, Medicaid waiver programs, and community organizations.

This role will also help mentor and guide a junior marketer while assisting with business development and community outreach initiatives.

Responsibilities

  • Develop and grow Medicaid and private-pay home care referrals

  • Build and maintain relationships with:

    • Hospitals

    • Skilled nursing facilities

    • Assisted living communities

    • Rehab centers

    • Social workers

    • Case managers

    • Physicians

    • Senior centers and community organizations

    • Medicaid waiver coordinators

    • Managed care organizations (MCOs)

    • IDoA and community agencies

  • Conduct in-person marketing visits and presentations

  • Coordinate community outreach and networking activities

  • Track referral sources and conversion outcomes

  • Assist with intake coordination and referral follow-up

  • Help develop marketing strategies for census growth

  • Mentor and train junior marketing staff

  • Represent the agency professionally within the community

Qualifications

  • Minimum 3+ years of experience in home care or home health marketing

  • Proven experience growing Medicaid and/or private-pay census

  • Existing referral relationships strongly preferred

  • Knowledge of Illinois Medicaid waiver programs and managed care plans preferred

  • Knowledge of the Chicagoland healthcare market preferred

  • Excellent communication and relationship-building skills

  • Strong organizational and follow-up skills

  • Ability to work independently and meet growth goals

  • Reliable transportation required
