Job Description
LPN - Long-Term Acute Care

Schedule:
8H Evening shifts, with a minimum of every other weekend.

Assignment Details:
Contract Length: 6 weeks
Guaranteed Hours: Not guaranteed

Requirements:
- Active LPN License
- AHA BLS Certification
- Minimum of 1 year of LPN experience
- Experience covering CNA duties as needed
- LTC experience preferred

Roles & Responsibilities:
- Provide direct patient care and support as part of a healthcare team.
- Administer medications and perform treatments as prescribed.
- Monitor patient health and report changes to the healthcare team.
- Assist with activities of daily living and ensure patient comfort.
- Collaborate with nursing staff to implement care plans.
- Participate in patient education and support their wellness and recovery.

Compliance / Notes:
- 7-year background check required within 18 months of start date.
- FACIS 3 & NY state check required within 30 days of start.
- Public transportation is not accessible; reliable transportation is recommended.
- Orientation and scheduling details will be provided.
