Responsibilities
- Perform comprehensive PT evaluations for residents and develop individualized treatment plans.
- Deliver evidence-based interventions, including gait training, therapeutic exercise, range-of-motion, and functional activity training.
- Monitor progress, adjust plans of care, document outcomes, and communicate updates to the interdisciplinary team.
- Provide patient and family education, discharge planning, and safety coaching to optimize independence and reduce fall risk.
- Maintain accurate documentation in the electronic medical record and comply with regulatory and facility policies.
- Collaborate with therapy aides and nursing staff to ensure safe, effective delivery of services within a 5-day workweek.

Requirements
- Active Physical Therapist license in Washington state (or eligibility for licensure in WA)
- Doctor of Physical Therapy (DPT) or equivalent; eligible to practice
- Ability to work in a long-term care / skilled nursing setting
- Experience in geriatric rehabilitation or SNF settings preferred; travel experience valued
- Availability for a 19-week assignment; schedule includes 08:00–16:30 shifts
