Physical Therapist

Position: Physical Therapist (SNF Setting) Salary Range: $45.99-$50.54 per hour Schedule: Full-time, 40 hours per week, Monday-Friday with no weekend rotation Facility: 111 Bed Skilled Nursing Facility We are currently seeking a highly skilled and dedicated Physical Therapist to join our team at our 111 bed Skilled Nursing Facility. As a Physical Therapist, you will work with a diverse patient population to provide individualized treatment plans and help patients achieve their maximum potential for physical function and independence. Key Responsibilities: Conduct comprehensive evaluations to assess patients' physical abilities and limitations Develop and implement individualized treatment plans based on patients' needs and goals Utilize a variety of therapeutic techniques and modalities to improve patients' strength, mobility, balance, and overall physical function Collaborate with other healthcare professionals to ensure coordinated and effective care for patients Educate patients and their families on proper techniques for performing exercises and activities at home Document patient progress and maintain accurate records in accordance with facility and regulatory guidelines Participate in interdisciplinary team meetings and contribute to patient care planning and discharge planning Stay up-to-date on current trends and best practices in physical therapy through continuing education and professional development opportunities Qualifications: Valid state license to practice as a Physical Therapist Minimum of 1 year of experience in a SNF setting preferred Strong clinical skills and knowledge of therapeutic techniques and modalities Excellent communication and interpersonal skills Ability to work independently and as part of a team Compassionate and patient-centered approach to care Basic computer skills and ability to learn and use electronic medical record systems #J-18808-Ljbffr Trinity Healthcare Talent Services
