Your Responsibilities

• Administering thorough patient evaluations, facilitating diagnostic procedures, and crafting personalized care regimens
• Identifying and addressing acute, episodic, or chronic health challenges in collaboration with physicians or independently based on your qualifications
• Educating patients and their families on disease prevention, wellness, and effective disease management
• Prescribing necessary medications and therapies within the limits of your practice
• Engaging in ongoing patient follow-up and chronic care management efforts
• Collaborating with interdisciplinary teams to ensure comprehensive, holistic care
• Accurately documenting patient information within the electronic medical records (EMR) system

Educational Background

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Graduation from a recognized Nurse Practitioner program

Essential Certifications

• Valid Nurse Practitioner license in the state of Utah
• National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or an equivalent specialty certification
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications, or the ability to obtain within 90 days
