Social Services Director

We are currently searching for a Social Services Director. Job Summary Responsible for the provision of Social Services to residents of the facility and their families under the direction of the Social Services Director. Must have working knowledge of interaction with residents of this facility. Responsibilities include meeting new residents upon admission, completing an Initial Psychosocial Assessment, visiting residents to follow up on concerns, and may include completing some MDS assessments under supervision. Arrange to purchase needed items for residents and ensure residents have adequate clothing items. Schedule visits to medical providers (dental, medical, podiatry, and audiology). Provide support in times of loss to residents and families. Assist residents in gathering supporting documents for services such as Medicaid, Medicare, SSI, etc. Perform other duties as directed by the Social Services Director. Must have the following: Basic knowledge of the operation of long-term care facilities, including the effects of disease, loss, loneliness, and boredom on residents, families, and friends. Knowledge of available community resources. Ability to work within a team and respond positively to directions from the head of the Social Services Department. Interest in a variety of age groups who comprise the facility population, with the ability to positively influence the lives of residents and their families. Ability to communicate effectively, with strong verbal and written skills. Qualifications The social worker must meet the following qualifications: Minimum of a Bachelor’s degree in social work; OR Minimum of a Bachelor’s degree in a human-services related major (e.g., sociology, gerontology, psychology, rehabilitation counseling, or special education); AND One year in a healthcare setting as a supervised social worker who works directly with individuals.
