Now Hiring: Physical Therapist (PT) – Home Health PRN

What You'll Do

• Perform comprehensive PT evaluations and build individualized treatment plans
• Deliver skilled physical therapy in line with physician orders and agency policy
• Track patient progress and adjust care plans as needed
• Teach patients and caregivers safe techniques, exercises, and equipment use for lasting results
• Document visits and clinical findings accurately and on time
• Flag changes in patient condition to physicians and the care team
• Provide clinical supervision to PTAs
• Take part in case conferences, care planning, and in-service education

What You Bring

• Degree from an accredited Physical Therapy program
• Active, unrestricted PT license in good standing
• 1+ year of physical therapy experience
• Home health experience a plus, not required
• Current CPR certification
• Valid driver's license, reliable transportation, and auto insurance
• Strong clinical judgment, communication, and documentation skills
