Continuum Therapy Partners is seeking a compassionate and dedicated Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our rehabilitation team at Randolph Health Services in Randolph, Wisconsin.

This part-time opportunity offers the chance to provide high-quality physical therapy services in a supportive, collaborative skilled nursing facility where your clinical expertise makes a meaningful impact every day.

As a member of our rehabilitation team, you’ll work with adult and geriatric patients to improve mobility, strength, balance, endurance, and overall independence while collaborating with an experienced interdisciplinary care team.

Responsibilities

• Provide skilled physical therapy services within your licensed scope of practice.
• Evaluate and/or treat patients based on individualized goals and treatment plans.
• Assist patients in improving mobility, strength, balance, coordination, and functional independence.
• Collaborate with physicians, nursing staff, rehabilitation professionals, and caregivers.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Communicate patient progress and recommendations with the rehabilitation team.
• Deliver compassionate, patient-centered care focused on positive outcomes.

Qualifications

• Graduate of an accredited Physical Therapy or Physical Therapist Assistant program.
• Current Wisconsin license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA) required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
