Highlights Pay: $54,095 - $116,760/year Location: Remote, GA Residency: Candidates must reside in the Southeast Region (FL, GA, AL, MS, NC, SC, TN, OH, AK, KY, VA) Schedule: Full-time What You'll Do Facilitate the delivery of appropriate benefits and healthcare information. Develop, implement, and support health strategies and policies. Act as a liaison with members, employers, providers, and insurance companies. Implement and coordinate case management activities for catastrophic cases and chronically ill members. Assess and analyze medical and vocational status of members. Communicate with members and stakeholders regarding care and treatment. Prepare documentation of case work activities. Consult with internal multidisciplinary teams for optimal health outcomes. Provide educational and prevention information for best medical outcomes. Monitor member progress toward desired outcomes. Requirements Education: Associate's Degree in Nursing (REQUIRED) Bachelor's Degree in Nursing (PREFERRED) Experience: 3+ years clinical practical experience, preferably in diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac, home health with Medicare members. 2+ years case management, discharge planning, and/or home health care coordination experience. Licenses & Certifications: Active and unrestricted Compact Registered Nurse (RN) license and live in compact state.
