- Conduct patient evaluations in inpatient and outpatient settings, tailoring personalized treatment plans
- Implement physical therapy interventions aimed at improving mobility, strength, balance, and daily function
- Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimal care and outcomes
- Educate patients and their families on therapeutic objectives, discharge strategies, and home exercise regimens
- Maintain accurate documentation of evaluations, treatments, and patient progress according to facility and regulatory standards
- Engage in interdisciplinary team rounds and conferences to facilitate coordinated patient care
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
- State licensure as a Physical Therapist
- CPR certification preferred
- Specialty certifications (e.g., neurological, geriatric) are advantageous but not mandatory
- Previous experience is strongly preferred
- Ability to effectively manage patients with intricate medical and mobility concerns
- Excellent communication and teamwork abilities
- Familiarity with digital charting and clinical documentation systems
