Oncology Inpatient Pharmacist – Acute Care Hospital | Roles & Responsibilities

• Participate in multidisciplinary oncology rounds and provide evidence-based medication recommendations.
• Review, verify, and optimize chemotherapy and supportive care medication orders.
• Develop and maintain chemotherapy treatment plans and protocols using EPIC/Beacon .
• Monitor medication therapy for safety, efficacy, drug interactions, renal/hepatic dose adjustments, and adverse drug reactions.
• Coordinate transitions of care between inpatient and outpatient oncology pharmacy services.
• Submit and manage non-formulary medication requests.
• Coordinate specialty medications and ensure timely medication access for oncology patients.
• Collaborate with physicians, nurses, advanced practice providers, and other healthcare professionals to optimize patient outcomes.
• Provide medication education and clinical consultation to patients and the healthcare team.
• Maintain accurate clinical documentation and comply with hospital, regulatory, and pharmacy practice standards.
• Participate in the on-call rotation as assigned.

Required Documents / Certifications

• Active Pharmacist License
• BLS Certification
• Minimum 3 years of hospital pharmacy experience
• Clinical inpatient pharmacy experience
• Oncology pharmacy experience (preferred)
• EPIC/Beacon experience (preferred)
