Medical Social Worker, Social Services, 40-Hours, Days, Monday - Friday, Every other weekend

Overview Full‑time position: 40 hours per week, Monday – Friday with every other weekend. Pay range: $25.05 - $33.05 a hour. The role reports to the Manager of Care Transitions and indirectly to the Unit Manager and Practice Leader. Disclaimer We are committed to equitable and transparent compensation practices. Responsibilities Care Transitions & Coordination: Reports directly to the Manager of Care Transitions and indirectly to the Unit Manager and Practice Leader. Kept department leaders abreast of any issues, trends identified and/or needs weekly and more frequently if needed. Demonstrated professionalism and teamwork. Worked collaboratively with the unit team, responded timely and efficiently, and covered for co‑workers during planned and un‑planned absences as requested. Provided service to the community through walk‑in and telephone inquiries as assigned. Conducted comprehensive psychosocial assessments for patients and families, identifying social, emotional, financial, and environmental barriers to care. Completed assessments fully, clearly, concisely, and within 24‑48 working hours of assignment. Documented using department policies, procedures, and state and federal regulations. Documented patient and family participation, multidisciplinary involvement, and other planning information as required. Discharge Planning Communication: Built rapport and responded to the needs of physicians, healthcare team members, third‑party payers, referral sources, and vendors to enhance customer service satisfaction. Conducted High‑Risk Screening on all patients on assigned units, completing the HRSN screen within 24 hours of assignment. Supported the discharge planning process by ensuring services and placements were appropriate, completing documentation accurately and in a timely manner, maintaining regulatory compliance. Educated patients on their rights and provided accurate, timely, comprehensive information to support informed decision‑making. Provided short‑term counseling to patients and families coping with illness, trauma, grief, or adjustment challenges. Ensured clear, relevant education regarding the care plan, tailored to individual needs, and collaborated closely with the multidisciplinary team. Responded to psychosocial crises including domestic violence, abuse, neglect, suicidal ideation, and family conflict. Advocated for patients’ rights, dignity, and access to appropriate care, addressing barriers related to social determinants of health. Conducted post‑discharge follow‑up on high‑risk patients to reduce rehospitalization. The Discharge Planning Process Completed discharge planning assessments timely, efficiently, and completely following regulatory standards and departmental policies. Appropriately leveled patients for home discharge with or without services or to facilities such as SNFs or acute rehab. Developed, coordinated, and implemented discharge plans on assigned cases with patients and/or family, identifying preferences and selecting HHA/SNF placements. Notified providers and established anticipated readiness for discharge, keeping families informed and documenting in EMR. Closed cases using appropriate transition‑of‑care communication forms timely and efficiently. Collaborated with the team to expedite discharge as part of the care transitions process. Completed case closures using appropriate communication forms in a timely and efficient manner. Maintained current knowledge and proficiency in discharge planning processes and best practices. Multidisciplinary Team Rounds: Participated in discharge planning rounds daily, collaborating with the team to determine each patient’s needs concurrently, addressing LOS issues, resources, and referrals in a positive professional manner. Utilization Management: Worked with the RN Care Coordinator to ensure appropriate and timely clinical information for utilization review with insurance payers, participated in weekly meetings, and initiated discharge planning at admission. Quality & Statistical Data: Participated in performance improvement activities and other projects, consistently completed statistical records for each closed case, captured hours and services, and maintained documentation for reporting and data integrity. Minimum Work Experience One to three years of experience in a hospital setting, skilled nursing facility, or community social/health agency. Minimum Licenses and Certifications MA State licensure required: LCSW, LSW, or LSWA. Required Skills Ability to work independently and with a team. Excellent verbal and written communication skills. Computer experience for data collection, report writing, and quality monitoring. Ability to work with community agencies to mobilize resources. Flexibility and adaptability to change.
