Community Care Physicians, P.C. is seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our Healthcare & Medical Services department within our dynamic, multi-specialty medical group. As part of a team of professionals dedicated to comprehensive, patient-centered care, you will provide advanced clinical services to a diverse patient population, supporting high standards of quality and innovation across various specialties. In this role, you will perform comprehensive patient assessments, obtain detailed medical histories, conduct physical examinations, order and interpret diagnostic tests, and develop evidence-based treatment plans in collaboration with supervising physicians as required. You will manage both acute and chronic conditions, prescribe medications and therapies within your scope of practice and state regulations, and coordinate care with other clinicians and support staff to ensure seamless, integrated services for every patient. You will serve as a key clinical partner in promoting patient education, prevention, and wellness, taking time to explain diagnoses, treatment options, and self-management strategies that empower patients and families to participate actively in their care. Working in a collaborative, team-based environment, you will interact closely with physicians, nurses, medical assistants, care coordinators, and other healthcare professionals, contributing to a culture of respect, continuous improvement, and innovation. You will use electronic medical records to document all encounters thoroughly and accurately, adhering to organizational policies, regulatory requirements, and best practices in clinical documentation. This position offers the opportunity to participate in quality improvement and population health initiatives, helping to shape and refine care models that advance patient outcomes and support Community Care Physicians’ mission of delivering high-quality, accessible healthcare. Depending on practice needs, you may provide care through a mix of in-person and telehealth visits, offering flexibility for both patients and providers while maintaining a strong focus on clinical excellence and safety.
