As an Advanced Practitioner at CapitalCare Family Practice Clifton Park, you will deliver full-spectrum family medicine services in a collaborative, team-based environment. You will practice to the full extent of your license with strong physician support, integrated clinical services, and scheduling designed to promote work–life balance and long-term professional satisfaction. Responsibilities

Conduct comprehensive patient evaluations and manage acute and chronic conditions

Order and interpret diagnostic studies, prescribe medications, and coordinate referrals as needed

Provide preventive care, health maintenance, and patient education

Collaborate closely with physicians, nurses, and support staff to ensure continuity of care

Maintain accurate, timely clinical documentation in accordance with organizational standards

Practice Highlights

Care across the lifespan – serving pediatric, adult, and geriatric populations

Structured five or four-day schedule available (8:00 AM – 4:30 PM)

with a consistent daily workflow and protected midday administrative/lunch time

Telemedicine visits and secure patient portal , enhancing access and continuity of care

Established, team-based environment

with multiple physicians and Advanced Practitioners supporting a strong patient panel

Convenient Clifton Park location , serving a growing suburban community in the Capital Region

Compensation: Starting at $125,000 for full-time

CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.
