Responsibilities
- Administer prescribed medications and treatments per physician orders and facility policy
- Monitor resident status and vital signs; escalate changes to the supervising RN
- Assist with activities of daily living, mobility, and personal care
- Perform wound care, dressing changes, and routine skin assessments
- Document care accurately in patient records and maintain care plans
- Collaborate with RNs, physicians, and the interdisciplinary team to coordinate ongoing care and safety

Requirements
- Active LPN license in Arizona
- Education: LPN diploma or equivalent
- Minimum 1 year of nursing experience; long-term care experience preferred
- Ability to work the night shift (3x12)
- Strong communication and teamwork skills; ability to work in a fast-paced setting
