Case Management Assistant II Per Diem

Organization: ABSMC-Summit Campus Position Overview: Coordinates and implements the transition of care (TOC) / Discharge (DC) plan for ambulatory patients. Prioritizes and coordinates the plan across the care of continuum through critical thinking, teamwork, and communication between care providers, patients, families and external vendors to ensure timely discharge. Pay Range is $33.67 to $43.76 / hour The compensation range may vary based on the geographic location where the position is filled.
