Summary: We are seeking an experienced and relationship-driven Hospice Clinical Liaison to serve as a trusted clinical resource to hospitals and other healthcare partners. You understand that excellent hospice outreach is not simply about generating referrals- it's about earning trust, providing meaningful education, recognizing when hospice may help, and making it easier for patients and families to access compassionate care when they need it most. You are proactive, responsive, comfortable starting conversations, and able to develop lasting professional relationships while maintaining the clinical integrity that hospice care requires. Qualifications: Graduate of an accredited school of nursing with a current Texas license as a Registered Nurse. Current certification in basic life support. Excellent organizational skills to handle a variety of tasks simultaneously. Effective interpersonal skills. Excellent communication skills, both oral and written. Ability to accommodate multi-faith beliefs and backgrounds of patients/families. Ability to function as part of an inter-disciplinary team. Ability to travel to multi-work locations during workday. Must have reliable, insured vehicle and valid Texas Driver's license. Duties/Responsibilities: Develop and maintain strong relationships with hospitals, physicians, case managers, social workers, skilled nursing facilities, assisted living communities, and other healthcare partners Serve as a clinical resource and subject matter expert regarding hospice philosophy, services, eligibility criteria, and the referral and admission process Identify patients who may benefit from hospice services and collaborate with referral sources to facilitate timely evaluation and referral Review available clinical information to help determine potential hospice appropriateness and coordinate additional clinical review when needed. Educate patients, families, caregivers, and healthcare professionals about hospice care, available services, and what to expect throughout the admission process. Respond promptly to referrals and inquiries and maintain consistent communication with referral sources throughout the referral process. Partner closely with Admissions, Clinical Operations, Business Development, and other internal teams to remove barriers and support timely access to care. Conduct educational presentations, in-services, and one-on-one education for healthcare professionals and community partners. Maintain an active presence within assigned hospitals, facilities, or accounts and develop trusted relationships with key clinical and operational stakeholders. Identify opportunities to strengthen referral relationships and increase awareness and understanding of hospice services. Ensure referral activities, documentation, and communications are completed accurately and in accordance with organizational policies and applicable regulatory requirements.
