Work Arrangement: This is a full-time, hourly employee position performed on-site. Schedule and workload are established based upon organizational needs, admissions volume, program operations, and the requirements of the position. The Behavioral Health Admissions Coordinator will work primarily from CRHE's Delaware location and will support timely coordination of referrals, intake, admissions, level-of-care placement, and clinical handoffs. CRHE provides outpatient behavioral health services, intensive outpatient programming, partial hospitalization programming, psychiatric medication management, integrated care coordination, and community-based partnerships designed to improve long-term health, recovery, and quality of life.

Position Summary
The Behavioral Health Admissions Coordinator serves as a central point of coordination for individuals referred to CRHE for behavioral health and substance use disorder services. The position manages the admissions process from referral receipt through intake coordination, eligibility review, documentation collection, level-of-care determination, admission scheduling, and handoff to the appropriate clinical program.

The Coordinator supports admission decisions within the scope of training, organizational policy, payer requirements, Delaware Division of Substance Abuse and Mental Health (DSAMH) requirements, and the American Society of Addiction Medicine (ASAM) Criteria. The Coordinator must be able to identify the appropriate CRHE service pathway for adults presenting with mental health disorders, substance use disorders, and co-occurring conditions and escalate cases requiring clinical review or a higher level of care.

CRHE's planned service continuum includes early intervention, outpatient services, intensive outpatient services, and partial hospitalization services. The Coordinator will work closely with licensed clinicians, counselors, psychiatric providers, care coordination staff, billing/credentialing personnel, referral sources, and community partners to support timely access to care.

Regulatory / Operational Classification
This position supports behavioral health admissions within a Delaware behavioral healthcare organization. The Coordinator must perform only functions authorized by education, training, experience, CRHE policy, payer requirements, and applicable Delaware requirements. Level-of-care recommendations requiring independent clinical judgment shall be reviewed by an appropriately qualified licensed clinician when indicated.

Essential Functions
Referral & Intake Coordination
• Receive, document, and track behavioral health and substance use disorder referrals.
• Conduct timely outreach to prospective clients, referral sources, and authorized representatives.
• Collect demographic, clinical, insurance, referral, and other information required to initiate the admissions process.
• Maintain an accurate referral and admissions queue and identify incomplete or missing information.
• Coordinate intake and assessment appointments with appropriate clinical staff.

Level-of-Care & Service Matching
• Review available clinical and referral information to identify the appropriate CRHE service pathway within the Coordinator's training and scope.
• Support placement across CRHE's planned continuum, including ASAM 0.5 early intervention, Level 1.0 outpatient services, Level 2.1 intensive outpatient services, and Level 2.5 partial hospitalization services, when clinically and operationally appropriate.
• Use approved screening and intake tools and the ASAM Criteria as required by CRHE policy and applicable program requirements.
• Escalate cases requiring independent clinical judgment, medical assessment, crisis intervention, withdrawal management, psychiatric evaluation, or a level of care not provided by CRHE.
• Coordinate referrals to external providers when CRHE cannot safely or appropriately meet an individual's needs.

Admissions & Documentation
• Coordinate completion of required intake and admission documentation.
• Verify that required consents, releases, identification, insurance information, and referral documentation are obtained.
• Coordinate benefits/eligibility verification with designated billing or administrative personnel.
• Track admission status from referral through scheduled assessment and admission.
• Ensure documentation is entered accurately and within organizational timeframes.

Clinical & Community Coordination
• Communicate with licensed clinicians and other members of the interdisciplinary team regarding admissions.
• Coordinate with hospitals, primary care providers, behavioral health organizations, social service agencies, and other referral sources.
• Support warm handoffs and continuity of care.
• Participate in admissions meetings, case review, and quality improvement activities as assigned.

Crisis & Safety Escalation
• Recognize information suggesting immediate safety concerns and follow CRHE escalation procedures.
• Coordinate urgent clinical review or emergency services when indicated.
• Document admissions-related safety concerns and actions taken.

Quality, Compliance & Access
• Maintain confidentiality in accordance with HIPAA, 42 CFR Part 2, applicable laws, and CRHE policy.
• Participate in chart and admissions audits.
• Track admissions metrics, referral conversion, access-to-assessment, and other operational indicators.
• Identify barriers to access and recommend process improvements.

Minimum Qualifications
• A bachelor's degree from an accredited college or university in social work, psychology, counseling, behavioral health, human services, public health, or another closely related social or behavioral science discipline.
• Master's degree in counseling, social work, psychology, behavioral health, or a related discipline preferred.
• Professional behavioral health license or license eligibility preferred, including LPCMH, LCSW, LMFT, LACMH, or another Delaware-recognized behavioral health credential, as applicable.
• Experience in behavioral health, substance use disorder treatment, admissions, intake, care coordination, utilization review, or a related setting.
• Working knowledge of the ASAM Criteria and behavioral health levels of care.
• Knowledge of mental health and substance use disorder terminology, screening, referral, and treatment systems.
• Experience using electronic health records and secure communication systems.
• Strong organizational, interpersonal, documentation, and communication skills.
