Licensed Practical Nurse (LPN)

Position Licensed Practical Nurse – Day Habilitation Program (Full‑time) Location West Springfield, MA, USA Hours Monday–Friday, 8:30 am–3:30 pm Compensation Hourly rate: $28.92. Sign‑on bonus: $500 ($250 after first week of training and $250 after 90 days). Summary The Health Services Specialist 3 is responsible for medication administration, G‑tube feeding, and other nursing duties as described in individual service plans. The role includes documenting all prescribed medication for members, assessing and attending to members’ health concerns and issues, and developing and implementing therapeutic programs. The LPN ensures programmatic and contractual obligations are met, works in accordance with regulatory and medical requirements, and provides supervision and training to nursing staff in identified areas. Responsibilities Medication administration and documentation. G‑tube feeding. Other nursing duties per individual service plans. Assess and attend to members’ health concerns and issues. Develop and implement therapeutic programs. Ensure programmatic and contractual obligations are met. Maintain compliance with regulatory and medical requirements. Supervise and train nursing staff as needed. Qualifications Valid Massachusetts LPN license. One year of human service experience. Valid driver’s license and reliable vehicle.
