LVN / LPN - LTC/SNF

Job Summary:

Responsibilities:

• 
  Administer prescribed medications and treatments accurately and in a timely manner.
  
  
• 
  Monitor and record patient vital signs, symptoms, and progress.
  
  
• 
  Assist in basic patient assessments and communicate findings to the registered nursing staff.
  
  
• 
  Maintain detailed and accurate documentation of patient care activities.
  
  
• 
  Ensure compliance with regulatory standards and facility policies.
  
  
• 
  Collaborate with healthcare team members to develop and implement patient care plans.
  
  

Qualifications:

• 
  Hold an active, valid LPN license to practice in North Carolina.
  
  
• 
  Possess at least 1 year of relevant nursing experience in LTC or SNF settings.
  
  
• 
  Demonstrate strong clinical skills and knowledge of long-term care procedures.
  
  
• 
  Exhibit excellent communication and interpersonal skills.
  
  
• 
  Ability to work collaboratively within a multidisciplinary team.
  
  
• 
  Ability to comply with facility policies and travel requirements.
