Senior Care Manager -Caregiver Services Program Salary: $62,000.00 - $65,000.00 Annually Application Deadline: N/A Nonprofit Provide information, assistance, prepare an appropriate caregiver support plan and coordinate services that will meet the existing needs for family caregivers. Organizational Values: The Community Agency for Senior Citizens (CASC) assists Staten Islanders age 60 and older to live as independently as possible in the community. Position Qualifications: Master’s Degree in Social Work (or related Master’s level degree) Minimum one year experience in social service organization Experience working with caregivers and/or older adults (preferred) Fluent in a second language (preferred) Requisite Skills and Capabilities: Excellent written and verbal communication skills, detail-oriented Collaborative: works with others in a spirit of trust and teamwork to establish and achieve common goals Sensitive to needs of older adults and caregivers Respects differences and values diversity; accommodates linguistic and cultural differences; works well with colleagues and clients with diverse ethnic and cultural backgrounds Self-disciplined: manages own time, meets deadlines, works well under pressure, and operates autonomously to achieve key outcomes Analytical: reviews options and considers implications prior to making decisions, develops systematic and logical procedures, presents clear and logical solutions Responsibilities: Conduct detailed intakes, initial in-home comprehensive assessments and re-assessment of family caregiver, including closing cases as appropriate Develop caregiver support plans based on presenting needs including but not limited to respite and supplemental services and coordinate services that will meet the needs Caseload expected to include more complex, high-need, high-risk cases With program supervisor/director, authorize services as appropriate and in keeping with eligibility requirements Maintain up-to-date knowledge of and working relationships with community resources Establish service linkages, make necessary referrals and monitor service delivery to successfully implement support plans Provide assistance to caregivers on obtaining access to available services and resources in the community. Provide advocacy services as appropriate Maintain up-to-date case records on all caregivers, including completion of necessary statistical documentation Maintain familiarity with all relevant NYC Aging standards, including updates to standards and procedures Assist with other program activities as assigned Additional responsibilities as assigned Key Results: Empowered and self-directed clients Effective and well documented support plans Effective referrals Effective implementation of NYC Aging and CASC procedures and guidelines Supervision and Education: Supervised by Caregiver Services Program Director Participate in team meetings Participate in NYSOFA, NYC Aging, and CASC training Participate in local educational and informational sessions and conferences Salary and Benefits: Full-time Generous PTO Pension Plan Supportive, growth-focused workplace Meaningful work serving older adults while empowering families and caregivers Compensation: $62,000-$65,000 Hybrid work options available post-training, subject to program needs and staffing coverage. #J-18808-Ljbffr Staten Island Chamber of Commerce
