Deliver PT interventions including therapeutic exercises, gait training, balance activities, range of motion, and pain management. Monitor progress, document outcomes, and adjust plans based on patient response and interdisciplinary input. Educate patients and families on home exercise programs, safety, and disease/condition management. Collaborate with the interdisciplinary team to support discharge planning, goal attainment, and coordination of care. Maintain accurate documentation in electronic health records and ensure compliance with regulatory and facility policies.
