Acute Care Nurse Practitioner Opportunity

This is an exceptional opportunity for an experienced Acute Care Nurse Practitioner to join our dedicated Heart Failure Center team at Ascension Saint Agnes, delivering comprehensive, evidence-based care within a supportive and highly collaborative cardiology department.

Join an established team of 10 physicians and 7 APPs. Call structure: None. Practice setting: Outpatient. Employed position within a nonprofit health system. Board certified in acute care. Eligible for state medical licensure and DEA registration.

Perform comprehensive clinical assessmentsincluding volume status evaluations and outpatient IV diuretic managementto optimize guideline-directed medical therapy (GDMT) for complex heart failure patients. Coordinate seamless post-discharge follow-up care and interdisciplinary treatment plans to strengthen continuity of care and prevent hospital readmissions. Serve as a clinical leader and mentor for Heart Failure Center RNs, collaborating closely with community cardiologists, pharmacists, and care coordinators to execute individualized patient care plans.

Minimum requirements you'll need:

• Nurse Practitioner with Prescriptive Authority credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date required.
• Registered Nurse credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.

Education:

• Master's degree of Nursing required.
