This role bridges the gaps in the transitional care and focuses on: Building referral relationships Supporting smooth hospital-to-home transitions Educating patients and families Coordinating post-discharge care Driving admissions growth within the assigned hospital systems Flexibility & Independence Hybrid work environment Autonomy in managing your day No overnight shifts or rotating hospital schedules Strong leadership support Help patients transition safely home Reduce hospital readmissions Educate families during critical care transitions Serve as a trusted resource for hospital teams Growth & Development Structured onboarding Mentorship and support Career advancement opportunities Free CEUs and ongoing education Key Responsibilities Hospital & Referral Relationships Build relationships with: Case managers Hospital staff Educate referral sources on: Home health services Programs and eligibility Concierge Home Care offerings Support referral growth and admissions goals Patient & Family Support Meet patients bedside prior to discharge Explain home health services and benefits Educate on: Insurance coverage Skilled home care services Transitional Care Coordination Coordinate care from hospital to home Collaborate with: DME providers PCPs Caregivers Partner closely with: Intake teams Clinical managers Schedulers Account Executives Provide timely referral and patient updates Ensure continuity of care throughout the admission process Referral & Documentation Management Process referrals in real time Complete admission documentation accurately and in a timely manner Track: Referral activity Conversion rates Qualifications Required Active Florida RN or LPN license Strong communication and interpersonal skills Detail-oriented and organized Valid driver’s license and reliable transportation Preferred Home health experience Hospital liaison or healthcare marketing experience Case management or transitional care experience Knowledge of payer sources and referral processes PTO starting day one Medical, dental, and vision insurance Employee Assistance Program (EAP) Supportive leadership and team culture Why Concierge Home Care?
