Nursing and Therapy for Home Health Care

We are seeking compassionate, dependable, and qualified healthcare professionals to provide high-quality in-home patient care. Team members will work directly with patients in their homes to promote recovery, independence, safety, and overall well-being while following physician orders, agency policies, Medicare guidelines, and state regulations. Positions may include: Registered Nurse (RN) Physical Therapist (PT) Occupational Therapist (OT) Certified Nursing Assistant (CNA/HHA) Registered Nurse (RN) – Duties & Responsibilities Perform patient assessments and develop nursing care plans Administer medications and treatments as ordered by physicians Monitor vital signs and patient progress Provide wound care, catheter care, injections, and other skilled nursing services Educate patients and caregivers regarding disease management and medications Coordinate care with therapists, physicians, and other healthcare professionals Complete OASIS documentation and clinical notes accurately and on time Qualifications Active Florida RN License CPR Certification Home Health experience preferred Valid driver’s license and reliable transportation Physical Therapist (PT) – Duties & Responsibilities Evaluate patient mobility, strength, balance, and functional abilities Develop individualized therapy treatment plans Provide therapeutic exercises and mobility training Assist patients in improving strength, balance, gait, and independence Recommend assistive devices and home safety modifications Educate patients and caregivers on exercises and fall prevention Document patient progress and treatment outcomes Qualifications Active Florida Physical Therapist License CPR Certification Home Health experience preferred Valid driver’s license and reliable transportation Occupational Therapist (OT) – Duties & Responsibilities Assess patient ability to perform daily living activities Develop therapy plans to improve independence and safety Train patients in activities of daily living (ADLs) Recommend adaptive equipment and environmental modifications Educate caregivers on patient support techniques Monitor progress and adjust treatment plans as needed Complete required clinical documentation Qualifications Active Florida Occupational Therapist License CPR Certification Home Health experience preferred Valid driver’s license and reliable transportation Certified Nursing Assistant (CNA/HHA) – Duties & Responsibilities Assist patients with personal hygiene and daily living activities Help patients with mobility and transfers Monitor and report changes in patient condition Assist with light housekeeping related to patient care Provide companionship and emotional support Record patient care activities according to agency guidelines Qualifications Active Florida CNA License or Home Health Aide Certification CPR Certification preferred Home Health experience preferred Valid driver’s license and reliable transportation Required Skills Excellent communication and interpersonal skills Compassionate and patient-centered approach Ability to work independently Strong organizational and time-management skills Knowledge of HIPAA and patient confidentiality standards Work Environment Patient homes and residential settings Frequent local travel required Flexible schedule may include weekends and holidays Equal Opportunity Employer Statement Our agency is an Equal Opportunity Employer committed to providing compassionate and high-quality care to all patients regardless of race, color, religion, gender, age, disability, national origin, or veteran status.aa415a4b-8b21-40fc-a65c-70d2b25ca29a
