As a Lead Care Consultant at Homematters - 029 Southeast Charlotte, located in Indian Trail, NC and serving the Charlotte Metro area, you will play a pivotal role in guiding families through the complex process of finding the best personalized care solutions for their loved ones. You will be the trusted advisor who listens, assesses needs, and connects clients with the most suitable home care services available. Your expertise and compassionate approach will ensure families feel supported and confident every step of the way.
Responsibilities- Conduct comprehensive assessments to determine client care needs and preferences.
- Provide expert advice and recommend appropriate home care solutions tailored to each family.
- Develop and maintain strong relationships with clients, caregivers, and healthcare professionals.
- Coordinate care plans and ensure seamless communication between all parties involved.
- Stay informed of industry trends, resources, and regulations to best serve clients.
- Manage administrative tasks related to care consulting and client records.
- Lead and mentor other care consultants as needed.
- Proven experience in care consulting, social work, nursing, or related healthcare roles.
- Strong interpersonal and communication skills with a compassionate approach.
- Ability to assess client needs accurately and develop tailored care plans.
- Familiarity with home care services and local resources in the Indian Trail, NC and Charlotte Metro area.
- Excellent organizational skills and attention to detail.
- Capability to work both independently and as part of a collaborative team.
- Valid driver’s license and reliable transportation preferred.
