Responsibilities Conduct social work assessments to identify and address the physical, emotional, and psychological needs of residents Develop and implement care plans to address the identified needs of residents Collaborate with the interdisciplinary team to provide comprehensive care and services to residents Provide counseling and support to residents and their families to promote emotional well‑being and independence Maintain accurate and timely records of resident assessments, care plans, and progress notes Participate in care conferences and other meetings to discuss resident care and services Stay up‑to‑date with industry developments and best practices in social work and gerontology Requirements Bachelor's degree in Social Work or related field Minimum of years of experience: 2 License or certification in Social Work Additional Information Salary: $85,000 Benefits: Insurance / PTO Job Type: FULL TIME #J-18808-Ljbffr
