Intake/Access Case Manager

Job Title: Case Manager FLSA Status: Full Time - Non‑Exempt Reports To: Lead Access Intake Case Manager Schedule: On‑call up to 40 hours weekly Rate of Pay: $26.00 hourly Closing Date: 4/15/2026 Benefits Health, vision, dental, life insurance, including voluntary life and disability coverage Sick leave: 1 day per month (12 days per year, accrual and availability begin at hire) Vacation: 10 days per year, accrued at .0385 hours per hour worked (no overtime), usable after six months of employment One floating day off, accrued immediately and again annually (usable after three months of employment) Pension plan, after one year of continuous service Voluntary tax‑deferred annuity plan (403(b)) Position Summary The Connection Center Per Diem Case Manager supports the provision of Coordinated Entry of social services in Colorado by providing information, referral, intake assessment, and emergency casework services over the phone, via email, video chat, and/or in person. The Manager will use homeless diversion and prevention strategies and will document assisted households in HMIS and Salvation Army databases for at least one year. Services target families in Metro Denver experiencing homelessness, at risk of homelessness, or precariously housed to promote long‑term housing stability. Hours: M‑F 7:00–9:00 pm, weekends 8:00 – 12:00 pm (extended in cold weather activation). Duties and Responsibilities Answer calls about Salvation Army assistance programs and community resources Triage, use VISPDATs, and provide assistance as needed Assess client needs and create individual case plans Connect qualified clients with appropriate partner agencies Provide tailored case management and financial assistance when appropriate Document calls accurately in the internal database and track assistance in HMIS per HUD standards Complete required assessments to determine program eligibility Use active, compassionate communication skills Maintain comprehensive case files and statistics required by funding sources Offer emergency services to clients by phone in Service Extension zip codes within the Intermountain Division where Salvation Army presence is limited or absent Education and Experience Bachelor's degree required (master's preferred). Degree in social work or a related field; transcripts required. Two years’ experience in providing casework services required. Experience working with individuals from diverse backgrounds and educational levels required. Customer‑service experience preferred. Working knowledge of integrated database applications preferred. Skills Critical thinking and sound judgment Team‑orientated Excellent oral and written communication Computer proficiency in Microsoft Word and Excel Adaptability to new software with basic training Bilingual preferred Qualities Highly motivated self‑starter Supportive of The Salvation Army’s mission Reflects and models the organization's high standards Requirements Must be able to drive. Minimum age 21, valid in‑state driver’s license. Annual MVR per policy. Background check: ongoing employment contingent upon biennial background checks per policy. Physical: able to maneuver, remain stationary, grasp, lift 25 lbs, operate telephone, access computer. Essential duties can be performed with or without accommodation per request.
