ensuring exceptional service from the initial assessment through ongoing care. You'll collaborate with caregivers, community partners to deliver high-quality, personalized care while helping grow our agency through outstanding customer service and relationship building.

Job Requirements:

• High school diploma or equivalent.
• Bachelor's degree preferred
• Valid driver's license with reliable transportation.
• Excellent communication and interpersonal skills.
• Strong organizational and time management abilities.
• Proficiency with Microsoft Office (Word, Excel, Outlook).
• Ability to work independently while collaborating with a team.

What a Client Care Manager will Do:

Client Care & Case Management

• Conduct in-home client assessments and develop individualized care plans.
• Coordinate the start of care and ensure services are delivered according to client needs.
• Perform routine supervisory visits and wellness check-ins.
• Maintain ongoing communication with clients, families, caregivers, and healthcare providers.
• Address client concerns promptly and ensure high levels of satisfaction.

Care Coordination

• Collaborate with the scheduling team to ensure appropriate caregiver-client matches.
• Monitor service quality and recommend adjustments as client needs change.
• Support caregiver communication and promote positive working relationships.
• Assist with onboarding new clients and coordinating transitions of care.
