Care Coordinator

Care Coordinator, Level IProvide comprehensive case management and rehabilitation services to adults with severe psychiatric disabilities in Contra Costa County. The care coordinator will coordinate and provide rehabilitation, linkage, and case management services to these individuals in their natural settings (community, homes, shelter, park, hospital, etc.) and at the office. Our consumers experience psychiatric hospitalizations, housing instability, homelessness, severe and acute mental illness, and money management difficulties. This program will take a do-what-it-takes attitude to support those we serve to find hope, stability, and recovery.Minimum requirements include:High school diplomaDemonstrate flexibility to handle diverse and changing organizational needsMust be willing to work full time, minimum 40 hours/week, five days a week, in the clinic and community. Must be willing to work evenings/weekends as needed.Demonstrate primary professional commitment and knowledge for representation of The Hume Center.Proficient in using GSuite (including Gmail, Drive, Sheets, Slides, etc), various Microsoft programs (including Word, Excel, and PowerPoint), and Electronic Health Records (EHR).Proficiency in Electronic Health Records preferredExperience working effectively with culturally, socio economically, and ethnically diverse populations.Possess a valid California Driver's license. Must be willing to use your own vehicle and valid car insurance for work related travel. This job requires travel between clinics and community sites in Contra Costa and/or Alameda Counties at times. Transporting patients in your car and sometimes in a staff van. Mileage reimbursement is paid for all work-related travel.Strong commitment to respecting consumers and their family members and having concrete ideas about how to improve the meaningful involvement emergency situations with supervisors and relevant colleagues.A history of maintaining confidentiality and discretion in all professional relationshipsFamiliar with and/or able to work within the Family Partner and Consumer Empowerment shift in Psychology.Ability to pass pre-employment background check, Physical, and TB clearancePreferred requirements include:Experience in a community behavioral health settingAA in Social Work, Counseling, Psychology or related field with several years' professional experience in behavioral healthCredentialed, or eligible to become credentialed, as a Mental Health Rehabilitation Specialist (MHRS).Proficiency in communication in any one of the following languages in addition to English: Spanish, Farsi, (Pushto/Dari), Punjabi, Hindi, Tamil, Urdu, standardsDirect services include:Provide services based on psychological and psychiatric assessments.Assist housing applications, support consumers getting into appropriate housing, and provide supportive housing services once consumers are housed to help them maintain housingProvide services in service program(s), included, but not limited to, evaluation, rehabilitation, case management, collateral, crisis intervention, and consultation.Complete case records, including progress notes, discharge/transfer summaries, etc.Gather information for evaluationPerform all evaluations, interventions, and outcome measurements on-time, including data entry of the results as needed.Help link consumers to SSI, SDI, Food, Housing, Transportation, Primary Care, Psychosocial Rehab, Educational and Employment Supports and Opportunities, and all other services that will help support the consumer's progress in treatment.Fulfill additional responsibilities as needed.Staff the crisis phone on a rotating basis with other staffOther duties include:Support and cover for other team members as necessary.Meet billable hours set by The Hume Center.Attend weekly staff meetings, trainings, group and individual supervision.Participate in quality assurance processes within the program and department and ensure all documentation meets all relevant internal and external standards.Community outreach.Meet with the Supervisor on a weekly basis for individual supervisionDemonstrate an understanding of The Hume Center's Philosophy, Organizational Culture, and Organizational Channels of
