LPN Job Summary: We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Meadow Falls of Conneaut, a premier long-term care facility dedicated to providing exceptional care and services to our residents. As an LPN at Meadow Falls of Conneaut, you will have the opportunity to make a meaningful difference in the lives of our residents and their families. Every other weekend and 12 hr shifts. Responsibilities Provide direct patient care, including administering medications, performing treatments, and assisting with daily living activities Develop and implement individualized care plans in collaboration with the interdisciplinary care team Monitor and report changes in patient condition to the nursing staff and medical team Collaborate with other healthcare professionals to ensure comprehensive care and support Document patient information accurately and thoroughly in accordance with facility policies and procedures Participate in ongoing education and training to maintain current knowledge and skills Demonstrate a commitment to providing excellent customer service and promoting a positive and respectful work environment Requirements Current LPN license Minimum 0 years of experience as an LPN Ability to work effectively in a fast-paced environment Strong communication and interpersonal skills Ability to lift, push, and pull equipment and supplies (up to 50 pounds) About Us: At Meadow Falls of Conneaut, we are committed to providing high-quality care and services to our residents and their families.
