Medication History Pharmacy Tech

Summary:
Medication History Technician: Obtains accurate, comprehensive medication history of patients. Interviews patient to obtain complete medication history. Communicates with outside sources to determine all current and active medications prescribed to the patient. Evaluates medication adherence based on patient interview. Accurately documents current medication history for physician reconciliation. This position reports to the Manager of Clinical Pharmacy Services and provides a vital service to our admitted patients and providers.

Credentials:
Essential:
* Pharmacy Technician License
* Certified Pharmacy Technician (PTCB)
* Pharmacy Technician (AAH)
* Nationally Registered Certified Pharmacy Technician

Experience:
Essential:
* 3-5 years related experience

Pay Range: $22.11 to $33.17 Based On Experience
