PART-TIME Nurse Practitioner (NP) – Assisted Living Primary Care | Flexible Schedule | Albany Region, NY

Location : Hoosick Falls, NY

Schedule: Part-Time | 2–3 Full Days per Week
Travel between assisted living facilities in Hoosick Falls and Cambridge

Compensation:
• Competitive Compensation (Open for Discussion)
• $5,000 Retention Bonus after 90 Days

Position Overview

A growing primary care practice is seeking an experienced Nurse Practitioner or Physician to provide on-site primary care services to residents in assisted living communities throughout the Albany region. This position offers flexibility, autonomy, and the opportunity to build meaningful relationships with a geriatric patient population.

Key Responsibilities

• Provide comprehensive primary care services to assisted living residents
• Perform patient assessments, diagnoses, treatment planning, and follow-up care
• Manage chronic conditions and preventive healthcare needs
• Coordinate care with facility staff, specialists, and family members
• Maintain accurate and timely clinical documentation
• Ensure compliance with Medicare and Medicaid guidelines

Required Qualifications

• Active New York State NP or Physician License.
• Currently enrolled and credentialed with Medicare and Medicaid (Required).
• Primary Care experience required.
• Active DEA Registration.
• Nurse Practitioners must have a minimum of 3,600 clinical practice hours and be eligible to practice
  independently under NYS regulations.
